Signed Letter from Employer: Not sure if it has to include local Consulate Address (Lagos)

Timeline

Our KI Visa was approved 09/08/2009 thank God and NVC is processing it already. I am trying to gather what I'm going to send to my fiance and it includes a letter signed from my employer according to the KI Flowchart

It must include job title, start date, salary and status (full-time) of your position

Now my question is does it have to include the Lagos address just like the example (Fiance Letter of Intent) below

Fiancé Letter of Intent

(petitioner’s / beneficiary’s address)

United States Department of Homeland Security

U.S. Citizenship and Immigration Services

(USCIS service center address)

or when submitting for interview

United States Department of State

United States Consulate, [name of country where Embassy is located]

[Embassy Address]

(date mm-dd-yyyy)

Dear Sir or Madam:

I, (applicant’s or beneficiary’s name) , do hereby state that I am legally able and willing to marry (petitioner’s or beneficiary’s name), and intend to do so within 90 days of my arrival into the US using the K-1 visa.

Lagos, Nigeria

Improved USCIS Form G-325A (Biographic Information)

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

What's wrong with "To whom it may concern"??? Or nothing?

Business letters customarily include the address of the recipient. Why wouldn't you include it if it's available to you?
