filling out the g-325a

my fiancee is trying to fill out on the g-325a, but she's come to a bit of a snag. she doesn't know whether or not to write her last name under "family name" or "all other names used" because her father didn't include the family name on her birth certificate. so should she include her actual family name or write her middle name as her family name as it appears on her passport?

Wow! Is this something that's unique to your fiancee, or this pretty common in Indonesia? Just wondering...

The safest bet is to use the name exactly as it appears in her passport. If she's ever included her last name for any purpose that might have resulted in some sort of record, then she should also list it under "all other names used". If she's only used it casually (like with friends) then she doesn't need to list it - it's just another name to get snagged in the name checks.

This probably should be in the K1 forum, since it's related to the initial petition filing.

she needs to write her name exactly like what she has on her birth certificate and her passport.

i also dont carry my dad's last name because my parents never put it behind my name, and it is pretty common in Indonesia, to answer JimVaPhuong's question:)

all other name used field is for maiden name or for any other name that she ever used in her life before she's married to you.
