I was wondering if anyone in Colorado, perferably Northern Colorado, has found a civil surgeon that will transcribe and sign the vaccination supplement form. I have heard that many will not, unless you go through the full medical, which I should not have to have a 2nd medical, as mine from the K-1 visa process is only 3 months old.

Does anyone have any names??

You can have this done much easier & cheaper by going to your local health department. Take any information you now have to them including the form for them to sign & stamp.

When I did this my health department wasnt familier with it but after some explanation they understood & filled in the form. The shots my wife needed as follow ups cost 23 bucks.

At out AOS interview they didnt even look at the form.

The local health department can administer any vaccination but if an updated vaccination record is required then the local health department cannot prepare the form...as it must be transcribed by a CS
