Yes. You need to have a USCIS approved doctor fill out the vaccination section and do boosters if necessary. Some doctors will try to insist on doing the full medical but this is not necessary. There is a Dr Arnold that will transcribe the records for you if you have the vaccinations completed but he lives in Texas. You can find his address here on VJ and he is really cheap. Better to do it now than get a RFE later. Good Luck!

YEP!

Get a civil surgeon to transfer the info from the DS 3025 to the I-693.

Dr. Arnold in Texas does the whole trip by mail and fax for $30.
