On form 485 it starts out asking Family Name, Given Name, Middle Name and then Address. Do we use my Fiance's old name (before we got married) and old address (from her country of origin)?, or do we use her new married name and my home address?

When I looked at the G 325A, it is the same form I already submitted in the beginning when I filed the I 129F. Are we actually supposed to submit that old previously submitted form again (it has her maiden name and address in her foreign country), or do I now complete it again with her new name and address here. When I looked at the G325A form on the USCIS web site (in the Adjustment of Status/Green Card section of the site), it has the same form repeated as you scroll down four times. Do we actually fill in the same information four times?

Thanks in advance to anyone who has done this and can help.

put the name on the I-485 you want the greencard to be in

update the g-325a for more current events (including change of name)... If you fill out the online version it will print the 4 copies automagically after you fill in the one page

YMMV

On I-485 use the name that is wanted to be on the green-card. There is a line further down on I-485 that asks for "Name on I-94" Madin name goes there. also G-325A has a line for "Other names used" this is for maiden name.

OUR TIME LINE Please do a timeline it helps us all, thanks.

Is now a US Citizen immigration completed Jan 12, 2012.
