It is form I-693 just the vaccinations part of this form needs to be completed by a US civil surgeon.

I got my medical examination in the Philippines. Per instruction of USCIS, KI visa holder must submit vaccination supplement when applying for AOS. What is this? Where can I get this?

It's your medical record that you have in SLEC.I hope CO will not ask for another medical here which is possible to happen even if your medical in the PI is still valid.

You need the form DS-3025 from your medical for a civil surgeon to transcribe the vaccinations on the I-693. The DS-3025 is the vaccination report from your medical.

If your medical was done less than a year ago then the exam part should be fine.

You don't need another exam ... with one exception ........THEY LOSE THE FIRST ONE.

Seems they are loosing a lot ot the X-rays and EXAMS from St. Luke. Don't ask me how or why I don't know it just seems they are.

So be ready for another $200 or $300 exam just in case .... it may not happen but it is happening to alot of us.

Most of my friends here on a K1 visa and me myself was asked for another medical even if our medical was still valid.

I just send DS-3025 without letting it transfer to I-693 and got approved!

8/09/08 --> "Pakage 3" in the mail

8/21/08 --> Medical Exam

9/19/08 --> Interview

9/26/08 --> Visa arrived

10/08/08 --> flying from Duesseldorf - JFK - Cleveland

11/08/08 --> finally married :)

12/27/08 --> AOS/EAD/AP send to Chicago

5/13/09 --> found a job and we picked out Molly (Beagle Puppy)

5/20/09 --> I485 "Card has been ordered" and again on 5/29/09

5/23/09 --> "Welcome to the United States of America"

6/2/09 --> AOS approved

6/5/09 --> GC in the mail and at night brinnging home Molly... such a great feeling :)

I just send DS-3025 without letting it transfer to I-693 and got approved!

If you had all the required vaccinations before your home country medical then you can send in just the DS-3025 and it won't be a problem, as above. If, however, you still needed to have any vaccinations then you need to get these and then have the DS-3025 transcribed to the new form with the new vaccinations added.

If your medical is still in date but you need the I-693 vaccination part filling out, you just need to schedule an appointment with a USCIS-approved civil surgeon. No new medical needed unless USCIS request it.

You are not required to have another medical examination as long as your form I-485 Application to Register Permanent Residence or Adjust Status,is filed within one year of your overseas medical examination.

You will, however, be required to complete Part 1,information about you,and submit the vaccination secton of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5 the Civil Surgeons Certification.

Some people have submitted a DS-3025 (obtained overseas at your medical documenting your vaccinations) with their AOS package and have NOT received RFE's. Some HAVE. If you want to be absolutely safe in terms of your medical documentation as a K-1 adjusting status, you should take your DS-3025 to a USCIS-certified civil surgeon and ask them to transfer the information from your DS-3025 to an I-693. They should do this for a nominal charge. If they try to tell you you need a full medical examination, DO NOT LISTEN. You ONLY need the vaccination part. The following memo from the USCIS documents that this completed form will remain valid beyond one year when submitted with most Adjustment of Status applications.

The civil surgeon should give you an I-693 documenting the fact that your vaccinations are complete (either they were completed when you had your medical or you had them completed stateside). You should submit this with your AOS package.

Why are K non-immigrant visa holders asked to include a vaccination supplement when filing Form I-485 to adjust status?

Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

Do I need to complete another full medical to adjust status?

Per Section 1245.5 in the Code of Federal Regulations, a medical examination shall not be required of an applicant for adjustment of status who entered the United States as a non-immigrant spouse, fiancée, or fiancée of a United States citizen or the child of such an alien as defined in section 101(a)(15)(K) of the Act and §214.2(k) of 8 CFR chapter I if the applicant was medically examined prior to, and as a condition of, the issuance of the non-immigrant visa; provided that the medical examination occurred not more than 1 year prior to the date of application for adjustment of status.

The vaccination supplement is the DS-3025 which was handed to you on the last day of the medical at St. Lukes. Some of the K1 applicants submitted only their DS3025 and didn't have any problems with it. Some got an RFE and was required to submit an I-693 signed by USCIS approved Civil Surgeon. It's your choice on what to send.

I just said what i just said based on my experience and some of my friends' experience. My Medical at St.Lukes was only 4 months old when i got here and was asked for another medical again here.. reason why i did not have an RFE which some if not most will have an RFE on this.
