20. Your fiancé(e) will apply for a visa abroad at the American embassy or consulate at:

If she is from Calgary do i put Calgary Canada ?

I know she is going to have to go to vancouver for her medical and to that embassy to get interviewed so what do we do ?

Montreal, or Vancouver whichever is closer. NVC will just send it to the consulate having jurisdiction over her residence.
