Our interview is coming up soon. And according to the checklist. they require the i-693 OR vaccination supplement (sealed package)

1) I only have D230 (Ivw sent a copy of this along with the AOS application) when I had my medical before my interview way back in October 2008. DO I still need to transfer my D230 to I-693?? or will the D230 still be acceptable?

2) They asked for a goverment issued ID. I guess this will be used when I enter the bldg. but I do not have any ID's except for my passport. Will they require me to leave an ID at the security or something? What should I do if I dont have one yet?

3) Do I have to go and change SSN now with my married name or can I do this after the interview?

The I-693 is required.

Passport will work.

What's a D 230????? Sure you don't mean DS 3025?

The I-693 is required.

What's a D 230????? Sure you don't mean DS 3025?

sorry yes i meant ds3025 i had included a copy of that form singed by the physician during my medical 6 months ago.i thought they had evrrything in that brown packet

Transfer it to I-693.

If you want to get an state ID you can get one at DMV. I had one now. I only showed my passport, SS Card with the present address (or any utility bills which your address is state in) and marriage certificate, told them to used my married name and that's it. I don't have to bring my passport.

In my case, I applied for SSN on my single name and after we got married I want to change it to married name but the person who assist me told me to go to Immigration and extend the I-94 because it was expired. She said there is no problem because I already had my SS number. I need to change it when I had the green card or the EAD.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. (this is what I had in mind) The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

"Q: Do I need to complete another full medical to adjust status?

Per Section 1245.5 in the Code of Federal Regulations, a medical examination shall not be required of an applicant for adjustment of status who entered the United States as a non-immigrant spouse, fiancée, or fiancée of a United States citizen or the child of such an alien as defined in section 101(a)(15)(K) of the Act and §214.2(k) of 8 CFR chapter I if the applicant was medically examined prior to, and as a condition of, the issuance of the non-immigrant visa; provided that the medical examination occurred not more than 1 year prior to the date of application for adjustment of status."

My medical is definetly not more than 1 yr and no RFE's. I just got my biometrics then interview letter tight away.

People have used just the DS3025 without any issues. (including my friend she just got away from it. NO RFE and got her biometrics). Good luck...

If your DS3025 is complete for all vaccinations then it may be acceptable to the person conducting your interview. If you did not get all immunizations prior to your visa medical exam, then you would have to get the remaining required

shots in the US and have the immunization section of the I-693 completed by a Civil Surgeon in the US. My husband got his GC issued recently with only DS3025, but his immunizations were all complete and listed on that form.

Photo id is to get in the door. A passport meets those requirements. You don't need anything else.

Your social security card does not have to be in your married name. I don't think it plays any role in the GC interview. After you get the green card, you can go change your name and have the restriction about working only with DHS authorization removed and a new card issued.

Thanks for the clarifications....yes all my immunizations are complete. Do I have to bring my xray on our interview? hehehe well im bringin it just incase.....
