Hi everyone,

My husband and I are getting the forms ready for his AOS and noticed he needs to send in a 693A -- supplemental documentation of immunation that needs to be filled out by a civil surgeon. However, we also have a copy of the "vaccination documentation worksheet" that was filled out by the embassy physician as part of his medical exam for his K-1 visa interview. I noticed that the form is pretty much identical to the 693A form (same information and questions)--just in a different format.

Does anyone know if our copy will suffice for the AOS or do we need a civil surgeon here in the US to basically copy the info onto the 693A?

The I693A is no longer used. The immunization section is part of the new version of the I-693. The instructions explain to have a civil surgeon fill out just that part if you're medical is under one year old. However, I decided to send the DS-3025 from the visa interview since no additional shots were needed. It worked for us. GC received this week. A lot of people call Dr. LE Arnold in Austin TX and send their immunization record and have him put it on the I-693 and sign off. He charges $30.

Thanks for the help! I see that he needs a follow up varicella vaccine...not sure if that is required for the AOS so will check on that. Again, thanks for your help!

Yes it is officially required for everyone (except <12m babies). However I sent a vaccination history that clearly showed I have NOT had the varicella jab. I received my greencard a couple of weeks ago, without ever receiving an RFE for the missing jab. Either I got lucky and slipped through the net or there are circumstances in which they turn a blind eye to that particular jab being missing.

YMMV, of course. My medical date was before the new rules surrounding the I-693 came into force so maybe that had something to do with it...
