Hello, guys! we are preparing for our papers for AOS hope to pass this week. I am answering for my G35A, I was confused in one of the questions here:

In second box here says" All other names used(including names by previous marriages)", what do I have to put in this box? Is this asking for my last name before marriage? please help me answer this, thanks.

Also in the fourth box, it says there: " Husband or wife( if none, so state, ____ and Family name ( for wife, give maiden name...

Yes, they want you to enter any names used prior to your current marriage, including your maiden or former name.

Yes, they want you to enter any names used prior to your current marriage, including your maiden or former name.

Enter your husband's name here.

In the 4th box you write HUSBAND then you put his surname where it says family name.

For previous names used, if you were married before put that surname and your maiden name (name before marriage) as well if you have used any other name you need to indicate it as well.

05/05/2009 - Received NOA2. Welcome letter for Permanent Residency. I-130 and I-485 approved 04/30/2009
05/08/2009 - Touched. I-485 approval letter sent
05/11/2009 - GC received in the mail. Expires 2019
05/11/2009 - Applied to remove restrictions on my SS Card
05/18/2009 - Received unrestricted SS card
10/13/2009 - My darling husband of 2 yrs 5 months 3 weeks 3 days passed away :(
