Depending on whether you have a K-1 or K-3 visa (or otherwise), the forms you need for your visa can be found and downloaded from here. Your Packet 3 should explain which ones you need to fill in.

2009-06-16: Received SSN

2009-06-18: Got my degree classification! Got a 2:1 =D.

2009-07-04: Got MARRIED on Independance Day in San Antonio!

:: AOS Timeline ::

2009-08-06: Mailed off our AOS packet!

2009-08-10: USCIS received packet, no NOA1 yet

2009-08-14: NOA1, eeee!

2009-09-08: Walk-in biometrics, all done!

2009-09-15: Got AP, woo + driving permit

2009-09-25: Approved for EAD, waiting for card

2009-09-30: EAD in hand :D

2009-10-16: Received interview letter for 20th October :D

2009-10-20: AOS APPROVED!

2009-10-30: Green card in hand! Weeeeeee!

Depending on whether you have a K-1 or K-3 visa (or otherwise), the forms you need for your visa can be found and downloaded from here. Your Packet 3 should explain which ones you need to fill in.

You don't send any of your documents/passport/etc back with Packet 3. You take them all to the interview with you. I was confused about the 'complete in duplicate' thing with the DS forms, so I just sent all 4 of them back in duplicate. Worked fine, haha.

So yeah. Just the forms and the checklist from the list Magpie posted :)

Yep, just the DS forms and the checklist. As long as you have the police report/other supporting docs before your interview you'll be fine :D

Yep, just the DS forms and the checklist. As long as you have the police report/other supporting docs before your interview you'll be fine :D

so i send it off and dont tick that i have to police report and that be ok?

we checked everything on the list (that applied to us) and sent it last monday, even though we are still waiting for the police certificate to arrive (we sent it out on 10 march. cheque was cashed on 17 march, so we are expected it soon.) and i havent sent the materials for the affidavit of support yet to my UKC (this week i hope.)

you really should/MUST have all of those things listed on the checklist (that apply to you) in hand at the interview. but i think it is generally acknowledged that it is okay to send back the checklist saying you do have them all, as long as you feel confident that you will have them by the time the interview rolls around.

i believe they will not schedule and interview without a complete checklist.

A cover letter is optional, I am sure they don't read them (at least if they read mine they ignored what it said). I just put all my documents in an envelope didn't worry about a folder. Do send it recorded delivery as a couple of people recently have found after 3-4 weeks that their packet 3s were not received. That way you know whether or not it was delivered.
