I recently received the noa2 :dance: and I need to get the most recent I-134 form to send to my fiancee.

I had a few questions

1. I checked on the uscis website and the latest I-134 form I could find there had an expiration date of 4/30/07 and a rev date of 02/28/07. On the visajourney site under the example forms the form has a expiration date of 5/31/10 and a rev date of 10/30/08. I was wondering how the visajourney site can have a newer version of the form than the uscis website. Is it ok if I use the version I found on the website or should I use the one I found on this website.

2. Does anyone know if this is the latest version of the form?

3. Can someone confirm that the I-134 needs to be notarized?

3. Do I also have to send a I-864 also?

If you click and download the one on the USCIS site notice it is also the updated form :dance:

Note over the weekend USCIS removed all links to I-134 and I-864 from the website, for some reason probably website maintanance.

The version above is still acceptable, however it will need to be notarized.

Is now a US Citizen immigration completed Jan 12, 2012.

No need to use the old form. Post 2 in this thread has a link from which you can the current form.

The instructions were incorrect and perhaps there was too many questions regarding the notary... There are, however, many other instruction pages that are still wrong so who really knows....

In any case, I gave you the link and you should be good to go. ;)

Another question, what address should the updated letter of intent need to have? Is the current letter of intent I have listed below ok?
