Timeline

Good morning!

The form I-485 ask for a Medical examination and says "...if you are a K1 or k2 dependent who had a medical examination within the past year as requiered for the nonimmigrant fiance visa, you only need to submit a Vaccination Supplemente with your adjustment of status application"

But we have seen many questions about Civil Surgeons to transfer de info from the Vaccination Workseet to the form I-693A.

Do we have to do that for Adjustment of status?

The vaccination supplement is a I-693A and yes you need to have this done for AOS.... call around the Cival Surgeons in your area and get the best price... you do not need a new medical... so dont let them tell you you do...

The vaccination supplement is a I-693A and yes you need to have this done for AOS.... call around the Cival Surgeons in your area and get the best price... you do not need a new medical... so dont let them tell you you do...

Where can I find the I-693A form? I was in www.uscis.gov but there is just the I-693 form ....

Maybe. Maybe not. My wife did not, and she isn't alone in that.

If the DS-3025 is not complete - all vaccinations either had or medically inappropriate - then you will need an I-693A prepared by a Civil Surgeon.

If the DS-3025 is complete then you may not need I-693A. It will depend on the luck of the draw, if the person who reviews your paper work considers I-693A to be essential without regard to a complete DS-3025.

You don't need to find an I-693A - a Civil Surgeon will have it.

My DS-3025 is marked as incomplete, I think the only vaccination I don't have is the Varicella. Does that mean I will need this vaccination before I submit the I-693A with the AOS package?

Yup, you need it, unless you had varicella, aka chicken pox, before, or have been significantly exposed to it to have an immunity. If you had it, for most civil surgeons, they will accept your word (as long as you remember when you had it or have records to prove it). If you are not sure if you had it, or think you may be immune, get your blood work (titer) done by your doctor and they can tell you for sure. The titer only takes a few days.
