Hello

I was trying to fill up I 693 and have soem doubts.

I am k1 holder and underwent a medical examination and the required vaccinations before my k1 interview.I had TDAP,MMR and Varicella vaccines and also have the report for that.

Anotther sheet from the doctor is abt X Ray and HIV

The doctor at that tiem did not fill up i 693 form.

I have the following doubts:

1) I am planning to apply for AOS ,AP and EAd together.

Do i have to send the I 693 form along with the above forms?

2) I understand that i have to fill up only pat 1 of the i 693 form and the rest has to be filled by a surgeon.How do i get that filled up and when do i have to do that?

3) Also,in the forum i have read that i have undergone the medical test within the past year,i dont have to do it again.I had my test in June 2007 .So what is it i have to do now

Could someone please explain the process

the foreign doctor would not prepare the I-693 (only filled out bu US Civil Surgeons) but would have prepared the DS-2053 (medical exam) and DS-3025 (Record of vaccinations)....

If the medical exam is less than 1 year old at the time of filing for AOS, you are not required to submit a full exam as they will attempt to use the one from the visa medical...

You can try to submit the DS-3025 in lieu if the I-693 Part II, Item 5 (vaccinations) and see what happens...

Otherwise you can visit a Civil Surgeon and have them transcribe the DS-3025 on to the I-693 Part II, Item 5 and then administer any missing vaccinations...

I just got a vaccinnation documentation worksheet (which says for use with DS-2053) and an X ray and HIV report.

I dont have any other forms

Should i just attach these or do i have to still attach i 693 form

It sounds like your "within one year" was up last summer in June 2008. If that's no a typo, then you have to have a full medical exam from a Civil Surgeon in the US. You can show him your immunization records so he knows you don't need those again.

It sounds like your "within one year" was up last summer in June 2008. If that's no a typo, then you have to have a full medical exam from a Civil Surgeon in the US. You can show him your immunization records so he knows you don't need those again.
