The I-693A who completes this form? Is this the form that is completed at the medical exam??

I think I read somewhere that if a medical exam was performed within the last year, which his was, that he would not be required to take another exam, but my main issue is vaccinations. The Dr at the medical exam never asked his about vaccinations and this was never asked about during his interview.

does anyone know how we can go about getting the I-693A filled out or how do we go about proving the immunizations he has had???

You need the vaccination section and section 5 of the form iI-693 filled out by a designated civil surgeon (look on USCIS's website for a database of civil surgeons). When your fiance/husband did his medical exam in Lebanon, he should have received a form from his medical package that lists the vaccinations he has had. If not, he should have brought evidence (medical histories, etc) that he has received the required vaccinations to complete the medical exam (not always the case, though).

Check out the USCIS's website, there is a link to the CDC's website that will answer this question and more. In sum, you need to schedule an appointment with a designated civil surgeon to have the I-693 vacc. and sect. 5 filled out (the I--693a no longer exists or is not required, just leave the other parts blank). To prove immunity one must have written proof (vaccination histories or a titres test (a blood test that checks for existing immunity))) or he/she can verbally state that he/she had acquired varicella (chicken pox) to prove immunity. by the osunds of your case, the quickest and most painless way to complete this form is to have a titres test completed, unless he has other forms of written evidence. You can set this up through the civil surgeon. personal doctor or at a medical clinic.

Does anyone know who can do the titres test? Can it be any lab or does it have to be a civil surgeon?

Has anyone "verbally stated" they have had vacc? How did you go about doing this?
