on the last part of the i-134 form. do i have to sign and give it to my fiance or i leave it blank ? or is this notarized ?? explain please. it says office administrating oath, so i guess thats the consulate officer right ?

I-129F NOA2 : 09-12-2008

NVC Received: 09-17-2008

Consulate Received : 10-10-2008

Consulate Mailed Packet to Fiance : 12-18-2008 - PACKET NEVER ARRIVED

Medical Exams In Juarez : 01-15-2009

Consulate Interview : 01-16-2009

Visa Finally Granted : 01-21-2009 - AFTER CONSULATE LOST PASSPORT FOR A FEW DAYS !!!!!!

POE at JFK Airport in New York : 02-01-2009

Timeline

Take it to your bank, they will notarize the form for free, but don't sign it yet, you sign it in front of the person that notarizes it.

If you take it to a bank that you do not have an account with, they will charge you a couple of dollars. If you take it to the UPS Store they will charge you $10.
