On Form I-765, Question Number 1 asks for your name. On the right part of that, it asks "Which USCIS Office?" and "Date." I am filing my I-765 along with my entire AOS Package in Chicago Lockbox Address so I'm not quite sure which USCIS Office this form is referring to. Is it the California Service Center or the nearest local office in my area which is in San Bernardino, CA?

If this is your first time filinf for the I-765 then you would not have an office or date, I believe that is for if you have checked the box saying you have previously filed.

That part on the right is not question #1. That is the continuation of question #11. Don't worry about it if it's the first time you are applying for EAD, leave it blank.

Yup, kid brooklyn is correct, just leave it blank. If this is the 1st time you apply, check NO on Ques# 11, and leave the following 2 questions blank, then proceed to Ques# 12.

09/12/08 Received Biometrics appointment letter

10/03/08 Appointment for Biometrics

11/04/08 Received Interview NOA by mail

11/10/08 InfoPass for AP

11/12/08 AP Approve Notice Sent , EAD Card Production Ordered

11/17/08 AP received

11/25/08 EAD received

12/11/08 Appointment for Interview - APPROVED

12/27/08 GREEN CARD received

ROC

11/16/10 USPS Express Delivery I-751 to California Service Center

11/17/10 USCIS received the package, Notice of Action dated same day
