Because each page goes to a different department. If you look at the bottom of each page, they are all slightly different. So yes, you have to complete all four pages.

While the information on them is the same, they each have different departments they'll be sent off to. It's a 4-part form (remember the days of carbon and/or carbon-less copies?).

There is a number to call the IRS to order tax transcripts. You tell them the year(s) that you need and they will send them to you free of charge. Takes a couple of weeks. Do a search on the IRS website to find the number.

Don't have as didn't keep (or can't find) copies? Or as in didn't file taxes?

If you filed but don't have/can't find copies - you can order Tax Transcripts from the IRS (they're free) and contain all supporting information so you won't need W-2s etc.

i got REF because of tax , my wife had extension for 2007 tax i sent 2004, 2005 and 2006 and they asked for 2007

When you call the IRS, try to get to a representative. They can fax you the transcripts right away if you ask nicely. Worked for me.
