I am no where near the AOS stage but I like been organied. If u get a medical done within one year from your AOS, i read that this iall you have to submit your D-3025 with your AOS forms you don't have to submit an I693 have it completed by a civil surgeon. Then I saw a thread the other day that said you had to get a civil surgeon to transfer the D-3025 for you to the I693 form as the I693a form is now obselete. Can someone please explain the procedure to me. I am confused as there are references that you have to get the Civil surgeon just to complete the DS 693 form what is this form?

Read the instructions of the I-693, Section III, Frequently Asked Questions, line #2. There you'll find the answer.
