Where on earth did you get your vaccination transferred to the I-693a. I just called the closest office and the girl has been there for 14 years and never had anyone do this.

She said I would need to have a blood pregnancy test to make sure I can receive the vaccines.....

Sigh...I am looking for more and have a whole list but I'll be on the phone all day.

Where did you guys go??

I am so upset I'm almost in tears. I've honestly called like 8 doctors, none of them do it without the medical.

There was a DR in TEXAS that would do it for $10! It seemed many VJ members would just mail him what was required!! Read that on VJ, so it must be true,lol No clue on how to find that link(s) maybe someone here does

Canadians Visiting the USA while undergoing the visa process, my free advice:

1) Always tell the TRUTH. never lie to the POE officer

2) Be confident in ur replies

3) keep ur response short and to the point, don't tell ur life story!!

4) look the POE officer in the eye when speaking to them. They are looking for people lieing and have been trained to find them!

5) Pack light! No job resumes with you

6) Bring ties to Canada (letter from employer when ur expected back at work, lease, etc etc)

7) Always be polite, being rude isn't going to get ya anywhere, and could make things worse!!

8) Have a plan in case u do get denied (be polite) It wont harm ur visa application if ur denied,that is if ur polite and didn't lie! Refer to #1

Isn't there a doctor you can mail your vaccine sheet to and they will transfer it to the "correct form" for about $20. I remember Spiritalright looking for the name of the doctor, maybe she can tell you. I was just thinking of taking my chances and using the vaccine sheet I received from my medical in Canada. I think people have had success doing that, though you might get an RFE.

To have your vaccination information transcribed to Form I-693, the following Civil Surgeon will do it by phone and fax for a nominal fee ($10 at time of writing):

I'm going to have them fill it out, better to be safe than sorry and get an RFE.

We are just sending the DS-3025 from the doctor in Calgary. I'm thinking if they've misplaced the medical, we are going to get an RFE for the entire thing even if we do get the vaccination record transcribed.

Just to make sure, Laura---you probably know this, but the 693A is now obsolete. There is a section on the DS-693 itself for the vaccines to go now. You can have him just do that section, plus the certification section, and fill out the "information about you" section yourself.

just to update , they moved to a different office, its now Suite 114 instead of Suite 106, and the fee is now 20$ :)

Yeah, I just found this out today.

Dr. Arnold's receptionist told me to send 2 copies of the first page of the 693 and just sign both. She was very clear about me not filling in any other information myself.

She wants all my info on a separate sheet of paper.

I'm assuming he will print out the other pages and then mail them all back to me. One sealed, one for my records.

I was told that I didn't have to do this because I was from Canada. I forget who told me but if you're from Canada you don't need to do this. So I didn't do this at all, hoped for the best and I was correct, I didn't get an RFE and everything was smooth.

Dammit I wish I could remember who told me this. :(

I've read the same thing on here too. Captain Ewok posted something about it back in 2006 and someone asked in 2007 if this was still the case.

In June of 2008 they came out with the new version of the I-693 and made the I-693a obsolete.

You mailed yours in before the change......I wonder if I could still get away with it....

I don't want to risk an RFE cause it says right on my sheet from Dr. Seiden "For Use in Canada Only"\

Should I just risk it?

No don't risk it if there's been changes. Plus you can get it transfered for a very good price, I would just get it done then.

Better over prepared than getting an RFE, I have never received an RFE and I don't think you have either, keep the record clean! :D

I just sent off my AOS package to the lockbox in Chicago. I found on the USCIS website that USCIS would accept my Canadian immunization records (from the civil surgeon exam) because the exam was done before a certain date. BUT, I don't trust these people (USCIS) to know their own rules and regulations. I used the guy in texas to transcribe my records too. They were very fast, I got it back in less than a week and they sent me a copy for myself and a sealed one for my AOS package. I agree, better to take the extra step, more because from what I've observed from other peoples experiences, people at USCIS don't know their own rules or have the ability to recognize the required documents so I decided to go the extra mile. I'm keeping my fingers crossed for no RFE's.
