HEllo VJers! I just got here in the US 2 months ago and Im in the process of strting my AOS. I didnt take my vaccination in the Phgilippines..my bad. However, as i was researching, I got confused about what kind of form to use since for K1 applicant in AOS, we only need to submit vaccination report, must I use the I-693 or the Supplemental (1 page) to I-693 Appendix A?

I went to the USCIS website and found this :

Q: If an applicant is required to receive only the vaccinations, and not undergo the entire medical exam, does he/she have to submit the entire Form I-693?

No. Applicants who are not required to have the entire medical examination need to submit only pages one, three and five of the new form. The required sections include Part 1, Information About You, the vaccination record portion of Part 2, and Part 5 (if the vaccinations are administered by a civil surgeon) or Part 6 (if the vaccinations are administered by a local health department). Pages two and four, that would have been left blank, do not need to be submitted.

But while reading some post here, I read that the 1 page I-693A Vaccination supplement form are used.

I hope those who have gotten their greencards or those who already filed their AOS can enlighten us.

some people just send the copy they recievd from the civil DR aboard , in my case I had my copy transcripted from DR ARNOLD

then send it with my AOS packet without any problems , good luck

Hi there I had same case as you. I didnt take the 2 vaccines in the philippines last march of this year. So i took all of the vaccines in here plus the HPV shot coz im stilli n the age bracket (9-26). Anyways, I used the I-693 form and filed out PART 1 (INFORMATIION ABOUT YOU) and then the civil surgeon filled out part 2 section 5 (PAGE 3 IMMUNIZATION PART) and Part 5 (PAGE 5 CIVIL SURGEON'S INFO AND SIGNATURE). THey will put it in a sealed envelope and give it to you. On top of that, I also included the vaccination worksheet from the philippines. Good luck to you:-)

the rules changed in june....

you need now the 693 with some parts completed ! BUT! it's not that easy !

I called all doctor in my area (LA) and not one accepted to do it so i had to go through (and pay) the full medical again + tb skin test + chicken pox (varicella) vaccination.

My advice is to just call doctors around and finding the less expensive (ask for the extra costs of vaccin aswell)

It takes about a week to have it after your first appointment.

Oh and yea. If you have your medical done in the philippines just recently like ON OR AFTER AUGUST 1, 2008, You will need to go through the tb skin testing and the varicella vaccination. BUt like my case my medical in the philippines was last March 2008 so its still good and I dont need to go through that again, although the VACCINATION PART I'm included on that because that's what I was lacking. So anyways im 26 years old and the vaccines that I have in here was MMR, Tetanus, HPV(gardisil) and Varicella titer not varicella vaccine for me because I already have varicella history (CHICKENPOX) when I was a kid so I still have immunity proven by the varicella titer that they got from me. GOod luck again to all of us :whistle:

Hi! Do NOT use I-693A, it is an old form! Use the I-693 (it is new) and follow the directions (complete part 1, part 2 number 5 and part 5 - see FAQs in the directions).

6/13/08 Applied for SSN for David ----- TRIP 1; 6/23/08 Went back to SSA 'cuz the person didn't pc all documents (rec'd letter from them) ----- TRIP 2

7/2/08 Received David's health insurance cards! Yay!

7/3/08 Went back to SSA; they said come back 7/9 if he still hasn't received it ----- TRIP 3

7/8/08 Got list of vaccines he still needed from CS by fax/phone; tested for immunity to MMR (only vaccine still needed) at our family dr

7/9 & 7/10/08 Went back to SSA, card still not received - Denied opportunity to apply for replacement ----- TRIP 4; Went back to SSA again to apply for a replacement card - Talked to a supervisor this time, POE people entered his last name incorrectly ----- TRIP 5; went to get David's ID thru the State, denied - instructions wrong on website; measles test inconclusive, another test needed; requested new lease

7/14/08 SSA called me in regards to problems at local SSA office

7/15/08 INFOPASS appt to fix POE name input error and then went to airport to fix POE name error

7/17/08 Received SSN card in mail and went to get state ID

7/21/08 State ID received in the mail and tested again for measles immunity

7/29/08 MMR shot #2 given and dropped off papers at Civil Surgeon (CS)

7/31/08 Picked up I-693, completed by local CS, (I paid $25, don't be fooled)

8/3 - 8/6/08 Worked on forms I-485, G-325A, I-864, I-765 and I-131; sent to Chicago - YESSS!

The I693A became obsolete in June 2008. You can no longer use it. If your vaccinations were completed in the Phils, you could have submitted the DS3025, but you will need to follow the instructions you copied from the USCIS use I693. No need for full medical if your full medical is less than 1 year old.
