Hi Vjs,

I have a piece of paper from St. Lukes hospital in the Philippines "Vaccination Documentation Worksheet", is this not enough and should i have to get I-693 from civil surgeon?

For my Adjustment of Status.

Thank you for your kind reply.

Make sure the "vaccination sheet" says that the history of the vaccines is complete. If you are missing any vaccinations you should visit a cvil surgeon and get them done.

I know...I paid $200. And the Civil Surgeon only needed to write on my vaccination part: applicant is pregnant, cannot receive vaccinations. It took him 5 minutes and he charged me $200.

hi deezee,

where did you have your medical done? i have been scouting around the san jose area and majority of the certified civil surgeons charge $100-$300++. this is totally absurd! why do we need to pay for another medical when we already paid st. lukes in manila an exorbitant rate for our medical? my husband and i are contemplating on whether to have the medical done or just pass the vaccination worksheet alone...pls tell me where you had yours done for $30...the name of the civil surgeon, his address and tel# too...thanks!!
