If you know what you are missing, then try your local public health clinic and then simply take the results to a CS to transfer to the proper forms

If no one with experience in the Denver area answers, the only way to find out is to call up the list of civil surgeons in your area, one by one, and finally go to the cheapest one! That's what I did for our area. Good luck! (And if you can't find anyone else, you can always contact the civil surgeon who completes the vaccination supplement paperwork for $10 or $15 by mail.)
