Hello. I am about to file my AOS and EAD application. I got my vaccination supplementary form transferred by Dr.Arnold. He sent me the form in a sealed envelop. Is there a requirement that says we have to submit this supplementary form in a sealed envelop? He sent me a copy but I'm a little concerned sending it in not confirming that everything was filled out correctly.

Did everyone hand in this form in a sealed envelop? Is there anywhere that says we must submit this in a sealed envelop? I know the brown envelop we got with the K-1 visa was not to be opened but I don't see anywhere that requires this supplementary form to be in a sealed envelop???

We received our vaccination supplement in a sealed envelope, along with a copy for our use. Most (maybe all) people give it in a sealed envelope, that is the standard way it is done (I guess supposedly so it is not tampered with). If I were you, I would just trust the copy you received is true and send it in the sealed cover. :yes:

Definitely don't open the envelope! If it is opened, USCIS may not accept it!

If I remember correctly it says so in the I-485 instructions.

We were handed our form from the Civil Surgeon. No envelope. We submitted it that way in our I485 packet, along with the receipt and the DS-3025 from the London Panel Physician. There were never any RFE's or questions about the form being 'unsealed'.

There is no requirement listed in the instructions for the I-485 that says the vaccination supplement has to be in a sealed envelope, but I know from personal experience that Dr Arnold informs clients not to open the envelope. You should have received a photocopy of the completed form, so you can see if there is any information missing. I'd be surprised if there was though, I get the feeling she does it a lot.

This is from I-693 instructions:

"The civil surgeon is required to give you the completed Form I-693 in a sealed envelope. Do not accept the form from the civil surgeon if it is not in a sealed envelope. USCIS will return the form to you if it is not in an envelope or if the envelope has been opened or altered."

While those instructions are for form I-693, I did some digging around on the CDC website who issues Supplement A to I-693 (also known as I-693a) and the CDC requires civil surgeons to put the appendix form in a sealed envelope too.

Interesting stuff.

From what I've seen . The I-693a is no longer supposed to be used.

i used dr arnold too and i sent the sealed envelope with our aos package.. i believe it's safe to do so.. :thumbs:
