What is the I-693a Medical Form. Do we need that for K-1 Visas for AOS.

I am ready to file for the AOS, however, why do some fillers submitt a sealed I-693a Envelope by the Civil Surgeon.

We don't have a clue what this is.

I do not think we received one on / or during or after the medical exam.

I-693a is a supplement to I-693. If you had your K1 application medical done within the year prior to submitting an AoS application, a new medical is not required, but proof of vaccinations are. The I-693a is where this information is recorded.

A USCIS approved civil surgeon will transcribe data from a DS-3025 if you have one from your medical, or will look at your vaccination history records to determine what you need/don't need.

This completed form needs to be submitted with your AoS package in a sealed envelope.

I agree that what you said is the way it is supposed to work. My wifes physical was 7 months ago. We submitted the vacination supliment but now they are asking for another physical. I felt if was easier to get another than to argue with them that we don't need it.

USCIS has been known to 'lose' documents, including medicals, and requesting them again. I suspect that is what happened in your case - the medical information became separated from the file and rather than find it they are just having you do it all over again.
