Hi folks,

I received a sealed packet from the US Consulate in Amsterdam containing the medical forms I must hand over to the doctor on the date of my medical (this Friday). The packet is taped shut and has a green marker over the tape to ensure I think it shouldn't be opened. Also on the packet the officials (I presume) have printed - "Must be handed over to the medical examiner" or something to that effect.

I have read elsewhere on the forum that I must complete these forms before I go in for the medical examination. Is this true? Should I open the packet and fill them out?

Why would you complete the forms yourself? The instructions are very clear, IMO. If it says to be handed to the medical examiner and it's sealed, all you need to do is follow the instructions.

I did not complete any forms for my medical exam, the Doctor did.

08.19.2011 - Biometrics Letter Received

09.12.2011 - Biometrics Appointment

01.27.2012 - Card production ordered

02.01.2012 - 10 year GC Received

07.25.2021 - N400 filed online

08.09.2021- Biometrics re-use notice

04.18.2022- Interview done at Minneapolis USCIS Local Office   ✔️ Received N-652 "Congratulations your application has been recommended for approval" during the interview.

05.19.2022- Oath Ceremony in MN

Don't open the envelop. There is nothing special in it, but the doctor will open it for you. It contains a few sheets to be filled out by the doctor and the x-ray center. It forms you can also find on the US department of State site if you search pretty good so I don't know why they made such a big deal about sealing it up, but oh well.....

I just post my medical experience under the Dutch treads in de subforum.......

The instructions per embassy can differ. Just strictly follow the directions given and everything should be fine.
