I am completing the DS-156 online, and have 2 questions:

1. Line 18 and 19 asks about the ex-spouse. It says "Spouse's full name (Even if divorced or separated)."

What if she's a widow? Do I still list her dead husband's name and DOB?

2. Line 39: Was this Application Prepared by Another Person on Your Behalf?

If I fill it out for her, do check "yes" then complete line #40?

1) Yes, I would fill it even if widowed.

2) Optional, this has more to do with having a third party fill the forms out like having a lawyer prepare them.

Is now a US Citizen immigration completed Jan 12, 2012.
