No DS-3025 to file with I-693A

Hi all,

I am now beging the AOS process and had a question. It is my understanding that if your medical is less than a year old, then it is not necessary to have another medical done for AOS. I am suppose to submit a DS-3025 with my AOS application. However, the doctors office (Dr. Chana) would not give me any copies of my medical and specifically advised me "not to bother calling for copies at a later date". I plan on taking my immunization records from Canada to a civil surgeon here in the USA. The doctor will transcribe my Canadian immunization records onto the appropriate form, and my medical that was done in Canada (that I handed in at POE) will be forwarded to the local office. Is this correct?? Is there something I am missing? Has anyone else been in this situation?

Alll I sent for AOS was the Supplement 693-A form signed by a U.S. Civil Surgeon. All she did was copy my immunization records from the 693 copy the Civil Surgeon that did my medical gave me

Well, Gee, wasn't that nice of the Dr's office :rolleyes:

It's too bad they didn't give that to you, or say they 'could' get you a copy.

All I did when I sent in my AOS was send in a copy of the DS-3025...didn't bother with the I-693A. Didn't get an RFE, got sent to California, and had the GC in 2.5 months.

I'd call them up anyway, and see what they say. You might get someone different...and you never know.
