Ok, this topic is quite old but I am just adding information for those who might need it in the future (that is, if they use the search function lol)..

Today I went to a civil surgeon on Oahu to have my vaccination details and TB skin test results transcribed onto the I-693. Total costs for me were $24.05 and it was done without any problem, no need for a new med. exam either.

The details of the civil surgeon are as follows:

Dr. Michael M Kusaka, Straub Beretania

839 South Beretania Street

Honolulu, HI 96813

Phone: (808) 522-4320

Aloha from Maui... (originally from Canada, Vancouver BC)

The way I understand it, the medical exam that you're required to do for 'VISA' is valid for 1 year.
