no place to list previous name on the I-485

Hello, I am reading through the checklist for filing for AOS and it says to use the non citizens married name and list previous name. I dont see a place to list her pre marriage name. I need to mail this packet in soon and I am getting very stressed out because I want it to be correct the first attempt. Can someone shed some light on this please?

I think part 3A on the form asks for the name as it is on the i-94, so if you changed name after entering the US your old name should show on the i-94. That's where you list the maiden name if so.
