You need to call arround, back when I had my wife's I-693 done, it took me visiting the doctor's office with a copy of I-693 instructions, and a copy of I-485 instructions with the appropriate paragraphs about K-1 vaccinations highlighted. Seeing that I he was not going to be able to hit me for a full medical, he quickly reduced fee to $30 to do just vaccinations record.

This service should cost you between $30 to $60 depending on doctor.

If you live anywhere near Joliet, IL (southwest suburbs of Chicago), I would recommend Physician's Immediate care on Houbalt Road. My husband had his I-693 vaccination transcription done here. It was $25 at the time, but I believe it's $45 now. They can also get you up-to-date on any vaccines you're missing. It turned out my husband needed one more round of MMR, and they gave him the shot then and there ($102.50 for the shot). Call (815) 823-8800. The civil surgeon's name is Jen.

Thanks guy's for all your answers......I got it done at Physician's Immediate Care on Cicero for $25, all my vaccinations were up to date, as i had them all done before i flew out.

I called Physician's Immediate care on Houbalt Road in Joliet today and they told me they no longer do vaccination transcriptions over there. Anyone had his vaccine book transcribed around willowbrook here in Illinois(60527). Thanks!
