My fiance was provided a Refusal Letter on 03/15/2024 at the consulate in Nairobi that was very vague. He failed his medical exam due to an apparent use of Khat. It reads, "Your visa application has been refused under Section 221(g) of the Immigration and Nationality Act (INA). For our case to be considered further by an officer, please submit all the requested documents in the specific manner requested below. Some may be sent via DHL while others will be uploaded through the CEAC portal (see instruction sheets provided)."
There is a handwritten X beside "Medical Examination from Migration Health Assessment Center. To book an appointment at IOM in Nairobi:" No further guidance was verbally given to him which left us confused.

My fiance attended his interview in Nairobi and was handed a very general document that stated that his K-1 Visa had been Refused. The document indicated that the reason which had a checkmark beside it was: "Medical." He was told that IOM would reach out to him regarding their decision. He had chewed Khat once and shared that with the officer during the interview. Apparently his medical exam revealed that Khat was in his system.
Just FYI, no cell phones are allowed in the consulate office. He had to pay a nearby third party to store it for 1000ksh until the interview was over.
