My spouse was in and out in about 1 hour and 15 minutes. Most of that was waiting. The interview itself was only 5 minutes. Things we learned:
1) Lots of confusing info on what to bring U.S. state website and embassy websites don't agree. Even the embassy would send emails telling us to bring certain documents and we'd call to clarify, and they'd say we didn't need to bring them. All we actually needed was:
Police Certificate
Her Japanese Passport
2 Pictures
Original Marriage Certificate
Self addressed Letter Pack Plus (don't get the Letter Pack Light version!!!)
Medical report was transmitted electronically from the Dr - so she didn't even need that.
That stated, she took everything (I-130, I-864, all of it). Given the importance, it's just paper and easier to have it in case.
2) It'll take 2 weeks to get your passport back while they apply the visa.

This is second hand information. Butttttt this is what I was told.
Check in was super easy.
They offered an interview in English or Japanese. My wife asked for Japanese. But her interview ended up being in English. She said the man was really nice but talked really fast. She said he seemed distracted and kept talking to someone else in the room(?). He only asked a few questions; how/where we met, when we met, etc. Nothing to difficult. The interview took 5-10 minutes. So the wait was longer than the interview.
The CO asked for my wife's Japanese & Chinese police certs, her birth certificate, and something else. (I forget what). He took her passport and a few days she got it back with the visa.
Overall she was quite happy with the experience.

Let me tell you how the interview went lol
My appointment was at 8:30 at US embassy in Tokyo. (It's way too early...I had to leave my house at 7)
The interview was quick and easy, the interviewer was serious and skeptical black lady.
She asked where my fiance and I met in person, I said the city where he lives.
She goes "really, why there?" I go "that's where he lives." She goes "there sucks, I'm from there, there's nothing lol"
I'm like okaay yeah it's suburban but close to big city...
She obviously love the lively busy atmosphere in Tokyo and hate suburban place.
She goes "so you guys only met three times in person?" I go "yeah but have been talking every single day for 5 years."
She goes "oookay (skeptical)".
Lastly she say "your visa was approved, hopefully you guys can move out from the city".
I laughed (I was like you reaaaally hate there lol)
I arrived around 8:20 and interview was done around 9:30.

Very professional and kind interviewer. He went above and beyond to expedite the K-1 visa after the interview.
