I do believe we are saying the same thing
in order to get it transcribed you must see a civil surgeon
they will let you know but you can also be prepared. Complete (as mine was marked in Canada) with a MMR shot in 1983 was not sufficient for the I-693 you have to have one more recently. If you are unsure whether or not you will need another vaccination, you can be certain the the civil surgeon will let you know

Yes Robert they received our the same day (4th) with no email/text as of yet...I sure hope I get that email/text soom my medical expires Oct. 13/10 ... I really don't want to have to do it again lol that would be a complete PITA!

LOL isn't this waiting fun (please note the sarcasm!)
We held off in filling for AoS, I'm thinking that was dumb but, oh well...Now though I have to admit I'm a little worried..Medical expires on Oct. 13/10 we have that it was in their hands on Oct. 4th but no email/SMS saying it has a number yet..Not going to get myself in too much of a panic if I have to redo the medical I do...
So I just thought I'd say hi!

No it needs to be transcribed and you have to have a recent/current MMR vaccination. Keep looking around for someone that will do it and cheaper. You do not have to do the full medical again but you might need an MMR shot. You will need to send in the sealed I-693 envelope though and they will give that to you when the civil surgeon transcribes the forms for you.

It seems the MMR vaccine is now a time sensitive vaccination in the USA. I had mine when I was 5 because you require it to begin school. NOW the US is making people redo the MMR vaccination to be recent/ current. So what we now have to do is take the original DS vaccination form to the civil surgeon along with a document from a clinic stating that you have had a 'new' MMR shot and they will fill out the required sections on the I-693 and then seal it in an envelope and give you a vaccination copy of I-693 for your records. The sealed envelope is what goes in the package. If you met all requirements for the K1/3 visa and age appropriate box is checked (as mine was) you will more than likely need another MMR shot documented on I-693.
All of this for a fee of course(my total was $50 new MMR shot and $10 to fill out I-693 and seal it). Do NOT let them do a complete medical again unless you do not have a copy of the DS form with vaccinations on it.

Make sure you pay special attention to the dates you had the required shots - tetanus with 5 years and MMR within 10 years. If that is not the case you will have to get those shots before going to the civil surgeon (or get them done at the civil surgeon) you will receive a new sealed envelope that will go with you AoS package
along with a vaccination copy for your records.

Yes transcribed it all onto I-693
and then sealed it in an envelope but gave me a copy of the vaccination part.

Strange...
I knew about the 'new' vaccination requirements of needing a 'new' MMR shot. So what I did was paid only for an MMR shot at a local doctors office. The doctor laughed and said I was the oldest person he had ever given the shot to. After I had the 'new' MMR shot I took the DS form to the civil surgeon along with the document of the 'new' MMR shot and had it transcribed for $10. I sure hope that was all I needed to do.. :S

Hi and welcome!
Please don't feel bad for not knowing where to look!! There is a lot of information on this site and it can get overwhelming. Just ignore the rude comments and take the helpful ones. There are good people here that actually do want to help, others well, I don't know why they are here commenting if they don't want to help. Seems odd to me.
Oh yeah and I couldn't find what touched meant but because you asked now I know so a BIG thank you for asking

Hi,
Hubby and I are sending off AOS package and we've followed the step by step guidelines completely (K3) but now I'm wondering about all the other stuff that isn't on the list...
evidence of ongoing relationship?
joint accounts/utilities/assets/lease?
Does this all go in the package being mailed or do we wait for interview for that stuff?
The only thing we have joint currently due to no SSN is our apartment lease. We did apply for the EAD ahead of time but that hasn't come through as of yet. We would wait but my year is up on my medical as of Oct. 13, 2010 and I don't want to have to do it again. (too much money lol)
Thanks,
Brandi

Am I correct in assuming this is for the 'Application for Immigration visa and Alien Registration' #40a?
I'm from Canada and had a very similar situation. I answered no because I'm not a harm to myself or others. With the question of medication you will have to have your doctor put the nature of the illness in perspective in letter form. Have him/her write on the letter that must go with you to the medical Symptoms are under control and not likely to reoccur (if that is the case). When I went to the interview, make sure you have health insurance document stating that you are covered medically in the USA. It is not a required thing but anything you have will help. I honestly had no problems. You can PM me if you'd like more information.

Thank you to everyone who helped get me home! Now we move into the AOS category...we might wait a few months just to enjoy the fact that the traveling has come to an end! POE was emerson/pembina ND....pretty smooth once they figured out what they were doing, they even had to pull out the manual to figure out how to use the computer system lol. All in all though took about 25 minutes. Easy compared to everything else. I'm all smiles!! K3 has worked in our favour to bring me home faster. CR1 I would have still been waiting. It might be a little bit more pricey but money is nothing in comparison to being with your loved one. My prayers are with all who are still navigating this process.
Thanks again!

Got it, took 40 minute drive and 5 minute wait to get a new one....and I might add it was $21 cheaper go figure!!...Darn Winnipeg!!

I'm in Manitoba and I was told that the police certificate didn't have an expiry date by the issuing agent. It was only after I realized it would be in my benefit to get another. I might try to see if I can go to a place outside of Winnipeg to get it done today?
-
I know Colorado is just a quick switch once you have your SSN they take a photo and mail you your new licence. Canadian licence is totally transferable.
But of course I can't do that yet as my interview is monday and it is the K3 as MTL is running 3 months later for us.....boo!

Hi,
I have applied almost 3 weeks ago for a new police certificate. I do have an old one from April 2008 in the file but the new one hasn't arrived and my interview is on Monday in Vancouver. The flight is already booked and can not be cancelled. I have every other possible piece of information they could possibly need. I do have the receipt that it is paid for but not received. What are my chances? Advice would be great even if it is not what I would like to hear.

Just curious my husband and I have been following the boards to see if we can find answers to our questions...I notice a lot of people can track there progress online? or by phone? or possibly by paperwork what is what we have been doing....we are in the same position as USA81 our paperwork was approved and sent off to NVC how do we know when it will get there? Do they mail out the case number? Do they have an online system like USCIS?
I'm in Canada and I travel back and forth to be with my husband until we are through this nightmare. Can I get the medical done in the states, oddly it is cheaper than going to Montreal for a day and then travelling again for the interview.
my husband has made over the poverty line for just coming up on a year will we need a sponsor?
I have a psychiatric condition that has been stable for over 4 years is this going to hinder me?
Just a few question to tag on...
