    Hey Michael & Amy,

    I didn't have to ask them for an extra copy. When I went to pick up my results, they handed me three things: the sealed envelop, a CD, and an envelop that was not sealed which contained a copy of my vaccination records (Vaccination Worksheet DS-3025). The nurse said she included that form in the unsealed envelop because I would need it when I AOS.
