Got it, waiting 8 months to get the re-entry permit is not an option for us then. In that case, I will try to limit the trip to 4 months and that should give a 2 month buffer.

1. Since it's her name, then there is no 'both names". List the one with the 4 names as it will cover the rest. People shorten their names anyway. But writing the whole name covers everything. 
 
2. You don't need the affidavit. But since you have it, you can send a copy. Won't hurt.
