    Also, looking over that form - most of the answers would be NA or 0 anyway since we haven't even lived in the US, so why would they want it from us?

    It’s confusing me too. We are actually looking for insurance plan now which are all expensive since I won’t be included to my Fiancé’s health insurance until next year

    Manila US Embassy states that a scanned copy of the signed I-134 is fine, if you don't have the original.
