    Thank you! Correct, he did not hold a J1 visa after 2015 and he returned to the UK before the required date all 3 years he held a J1 visa.

     

    In between 2015 and 2019 he did travel to the U.S. to visit me, but he did not have any type of work or personal visa. He only stayed for 2.5 weeks max any time he traveled to see me in that time frame. He just had an ESTA for temporary travel. 

     

    Does this information change anything?

  3. My husband is submitting for an adjustment of status from a K1 visa. He entered the U.S. from the UK on April 26, 2019 on the K1 visa.

     

    We are preparing his paperwork for the adjustment of status/green card and noticed on the USCIS website in the section where they have the checklist of documents required, it lists the I-612 as a required form if the green card applicant has held prior J1 visas.

     

    Here's the excerpt from the website:

    Documentation of past or present J-1 or J-2 nonimmigrant status (if applicable), including proof of compliance with or a waiver of the 2-year foreign residence requirement under INA 212(e) (for more information, see Form I-612, Application for Waiver of the Foreign Residence Requirement).

     

    My husband worked at a summer camp in New York State for 4 months (June - September) in 2013, 2014 and 2015. Each of those times he was issued a J1 visa from the U.S. government. He also received a U.S. social security number in 2013 when he applied for his first J1 visa.

     

    My question is: In this situation - does he need to include the I-612 form as part of his adjustment of status packet/application? Or would just copies of the previous J1 visas and stamps in his passport showing he left the country when he was supposed to do the trick?

     

    I read the instructions on the I-612 form and I'm still not clear on this.

    Bring the DS-3025 with you to the doctor but not sure if they take the NHS proof of evidence of the shot, but worst case you need to get that shot again...They are going to transcribe the DS-3025 to the I-693 and add your missing shots to the list.

     

    If you get the I-693 then you don't need to submit the DS-3025

    Follow-up question: Since my husband had the tetanus shot after his K1 medical and interview (but before entering the U.S. on the K1 visa) how can we prove this vaccine was administered? Of course it seems unhealthy/absurd for them to require he gets another tetanus shot just so it can say on paper that it was done by a civil surgeon, but it sounds like the USCIS does not accept "proof" of vaccinations from any doctor that is not a civil surgeon.

  6. We are applying for my husband's adjustment of status from a K-1 visa. He immigrated on the K-1 in late April 2019 from the United Kingdom.

     

    He got the pre-K1 interview medical exam in London and was missing 2 required vaccinations (flu shot and tetanus shot). Everything else about his medical was clean/approved. He booked in to the NHS doctor a few days later and got the tetanus shot, but he could not get the flu shot at the same appointment for administrative reasons.

     

    What is our best approach for getting the necessary paperwork for the I-693? Do we need to submit the DS-3025 that shows everything he has that's in good standing + having a civil surgeon administer the flu shot + review the confirmation paperwork for the tetanus shot given by the NHS doctor?

     

    Any advice appreciated. Let me know if I left out any information that would be useful.

  7. My husband is in the same situation (worked for three summers in 2013, 2014 and 2015) for 4 months at a time on a J1 visa at a summer camp in New York teaching martial arts. He's now applying for an adjustment of status/green card from a K1 visa.

     

    Did you have any issues with this? Any recommendations for what "proof" to show? Does he just need to show the copies of the visas from those years?
