    You have to specify your spouse information even you're are filing for waiver or if your spouse is deceased so fill out the part 4 of the form. Fill out the Part 7 and sign it. Leave the Part 8 blank.

    I am so sorry for your loss.
     
    I have gone through the same. Came to USA in 2014 and lost my husband in 2015, filed ROC waiver-spouse deceased and got the 10 year green card recently.
    I still have the copies of the forms i submitted with my ROC package.
     
    1. For Part 4 questions, I have filled my deceased husband's details here.
    his name, his dob,his ssn,his a number.physical address i added his last address and mentioned deceased last add.
    2. I have filled part 7 with my details, in part 7 question 6a. Petitioner's signature, put your signature.
    Part 8 Dont fill part 8. I left it blank.
    3 and 4. If you have document to support these then send a copy to them since account was closed only after he passed away and explain in cover letter.
     
    Hope this helps. Make sure you include a cover letter and explain everything and send as much evidence possible to show that the marriage was entered in good faith.

    I didn't send anything but the marriage certificate. I hadn't even thought to until reading about other people's applications much later. Had I known, I might have included a couple things but definitely not a whole slew as some have listed off here. From what I understand, all the financial/co-mingling stuff is a big requirement for ROC but not for AOS.

    No need to send all that with the initial filing. It is not required. Read through the I-485 instructions and see if it tells you to include that. Nope. The only proof of your marriage that is required is a copy of your marriage certificate. IF you have an interview, then take whatever documentation you have at that time. It may or may not be asked for.
    To answer your question personally. No. We sent only what was required by the form instructions, and were never asked for any other documentation to prove our marriage was legit, not even at the interview.
