Thank you guys i just dont remember if i filled up a form there in Philippines for my daughters ssn during the process of crba & us passport. I think there's no form given right? For ssn while applying crba & us passport.,, coz we called the ss office about it and told us that we just have to wait for the ssn to be mailed. But like i said i dont remember filing up for ssn...

Hi Im here again lol! Im just confused about this elis immigrant fee "about you". In our visa we only have Last Name and Given Name right? Given Name contains our Full Name and Middle Name right? In elis, they ask for Last Name, 1st Name(Given Name), Middle Name. It says that put X on No 1st Name and X on No middle Name if only Last Name appears on visa. It says also rhat copy the info/format on your visa. For example:

*In Visa

Last Name: Reyes

Given Name: Angela Mae Cortez

*which is correct for elis visa fee:

1. Last Name: Reyes

1st Name(Given Name): Angela Mae Cortez

Middle Name: X

2. Last Name: Reyes

1st Name(Given Name): Angela Mae

Middle Name: Cortez

The purpose of the medical is to look for communicable diseases (e.g. TB, leprosy). If you do not not have a communicable disease, then you have nothing to worry about.
