  1. Thanks for you input. I found a link earlier on this thread that had the following info:

    Per Section 1245.5 in the Code of Federal Regulations, a medical examination shall not be required of an applicant for adjustment of status who entered the United States as a non-immigrant spouse, fiancée, or fiancée of a United States citizen or the child of such an alien as defined in section 101(a)(15)(K) of the Act and §214.2(k) of 8 CFR chapter I if the applicant was medically examined prior to, and as a condition of, the issuance of the non-immigrant visa; provided that the medical examination occurred not more than 1 year prior to the date of application for adjustment of status.

    I added a supplement to the I-693 with the above info and commented that I had the medical, where & when. I attached the vaccination and xray report. Hoping for the best! I have the I-485 package completed, double checked (I found mistakes...always good to double check!) I am going to review the I-131 and I-765 in a bit and the package will be sent tomorrow :)

  2. Thanks, I will attach the other 4 to AP & EAD...I am now confused with the I-693 form, I have my vaccination sheet from my medical and a one page report of my xray...if we had our medical do we fill out section 1 and attach the evidence?

    Well, I was reading about the I-693, and there's a debate over it. I myself was (still am) confused about it. Some people say that we don't need it because we already had our medical, though some people got an RFE saying that they need a supplemental I-693 still, I guess meaning that their vaccines are not up to date. I went ahead and got a transcription through a civil surgeon, but I suggest doing more research about it. It cost me $30 for that transcription.

  3. Okay, I must be burnt out from paperwork...what am I missing...I had 6 current passport photos taken, I saw on the 1-485 that they want 2 passport photos, where do the other 4 go? :wacko: Also I want to clarify, do we mail this package to the Chicago Lockbox?

    Hi Lise,

    I just sent out my AOS application last Monday.

    1. The 4 other passport photos go to the EAD and the AP. So, 2 photos for I-485 / 2 photos for EAD / 2 photos for AP. (But, EAD and AP are optional)

    2. There are 2 addressed you can send your AOS to, depends on how you mail it: (This is in the USCIS website)

    It is 64 days from the start date to the end date, so you are still eligible based on your K-1 status, but your window of opportunity closes in 12 days.

    You either go back to that office to speak with a manager, or try another office.

    I did get my social security number, but this was before I got married. Now, I'm married and want to have my name changed with my husband's surname, but they told me that I need my green card first...

  5. Hi guys, I just need to straighten some things out.

    Here's my background:

    I entered the US via a K1 visa, and applied for Social Security. I got married last Sept. 2, 2011. I waited for my marriage certificate so I can have my name change done, since the lady we talked to when we first applied for SS said that when we get married, just go back and show the marriage certificate to have my name changed.

    Today, my husband and I went to the SS office to have my name changed carrying all my passport, BC and MC. However, the guy in the office said that we need to have my green card first before they can effectively change my name in my SS. Did someone have the same experience as I did? Is there any way I can get my SS or a proof that I can change my name with only my marriage certificate? I'm a bit bummed, because I wanted to have my Driver's license with my married name already so there won't be any hassle, as well as opening bank accounts etc.

    Thanks in advance for the help. BTW, I'm from the state of California.

  6. Where did you go? I'm in the SOCAL'ish area and need to find the right place in case we get an RFE. Sonoko and I decided to not do the I-693. If you just read the I-485 instructions, it is pretty vague, just saying to provide vaccination verification with the package. So hopefully the stuff from her medical will be sufficient!

    It will be interesting to see where this goes for everyone!!

    Mari... I wish you were a CS, I would make the extra travel for that! LOL So far I'm just not too happy about the ones around here! BLAH!

    Just an update on my process. My fiance and I will get married on the 2nd of Sept. Just a simple court wedding, because we will have a church wedding once we're able to save up.

    I found a civil surgeon who will do the transcription for $30, and that's the cheapest I could find here in SoCal. I went there, and they said that I don't need anymore vaccines. However, he told me to have a blood test to check if I've had chickenpox before. When I was in the Philippines, the clinic just saw the chickenpox scar on my face and concluded that I already had it when I was young. The civil surgeon said that they need an objective evidence, and that USCIS asks for it now. So, I just said yes, for $20. All in all, I had to pay $50, which I think is good because I called around, and the price range was $150-300. I'm just trying to make sure I don't get an RFE. I hope things will go by smoothly. I will be getting the sealed envelope on Monday. :)

    We are now on the process of fixing our AOS papers, and I'm looking for an affordable Civil Surgeon that will just transcribe. I have called around, and the cheapest one I could find was $150. Does anyone know someone cheaper and more affordable? I think paying $150 just to transcribe is way tooo much.

  9. I'm actually considering skipping the Civil Surgeon after reading http://www.visajourn...93-and-ds-3025/ :)

    My fiancee received these 4 items for her vaccination in China:

    1. International Certificate of Vaccination and Prophylaxis (A Yellow Book)
    2. DS-3025 form with check marks for the 3 shots she received, and a check mark for "Vaccination History Incomplete: Applicant may be eligible for blanket waivers because vaccinations not medically appropriate (as indicated above)" referring to booster shots not yet administered.
    3. A sealed envelop addressed to "USCIS (Vaccination Form)"
    4. A sheet titled "Appendix C. Vaccines To Complete for Adults".

    The above are in addition to the sealed medical report she had to turn in during her interview.

    I'm thinking item 3, the mysterious sealed envelop is as good as the Civil Surgeon's sign off. I also read somewhere at VJ (I don't remember where), that quite a few beneficiaries actually got their AOS approved without an I-693 form. They simply sent in what they received at their medical abroad.

    Since booster shots are not necessary for AOS, then item 4 is just for our beneficiaries' health benefit.

    I'm planning on sending color copies of the Yellow Book pages, a copy of the signed DS-3025, and the seal envelop addressed to USCIS.

    Anyone else care to share what their beneficiaries got with regards to medical and vaccination documents?

    I read somewhere also that going to a civil surgeon is not necessary. On the other hand, I am so adamant about not getting delayed. I'm so particular about wasted time. Grr! It's making me confused.

    Oh, I know. I'm dreading it myself...but not too badly. I think we are pretty well prepared. Have you guys got your SSN yet? I'll be working on Mr. Loxy's in about a week. Dreading the civil surgeon. ugh.

    Hi Dizzy, yeah I got my SSN yesterday, exactly 2 weeks after my arrival. Went by smoothly :) Just bring your passport, 1-94 (the small white paper immigration attached to the visa, and birth certificate. :)

  10. Yeah, it's already in the consulate. It's been there ever since they sent it last June. Just get the other documents needed for the interview, and also have your medical done. You can have the medical done anytime. It's only walk-in. Just make sure you don't have your period. Also, make sure to get your NBI clearance asap, because they are taking a longer time to release it now, since they're having computer problems. You can order your CENOMAR online or via phone. :) It's all in my blog site, just take a look at it. All the information is there. :)

    You don't have to wait for anything from the Embassy. Chances are, you won't receive anything from them. They should send out this so-called "Eligibility Letter", but I am done with my medical and interview, and I still don't have it!

    First, you can now pay for your visa fee at any BPI branch. Just bring your passport with you, probably also your child's. (I'm not sure about the children, since I went through the process without one. Just bring your child's identification, just in case)

    After 4-6 hours, you should be able to schedule. There are 2 ways: through phone or online. You can check out my blog, since typing it all here takes a long time.

    Second, you can also have your medical done already. You can have it done now, actually. Anytime between your interview. Look at my signature below, it should be there.

  12. My parents did their first day of medical today. They will come back tomorrow at 8AM. I hope the guard will say "for IMMUNIZATION" instead of "for SPUTUM"!. I am very anxious...

    I don't think having hemorrhoids will be a problem. What they are looking for is if you have any communicable disease. They'll probably just tell you to have it checked out.

  13. Don't regret it! All things happen in God's perfect time :) I understand how you feel though. Two days ago I attended my first military funeral, the death of a 24 yr old marine who was my husband's good friend. It really made me see how short and precious life is, and we must not take anything for granted. My husband is leaving me soon for a 14 month deployment which sucks, but I will just continue to have faith because I know God is looking out for us, and he is watching over you guys too :)

  14. Hi everyone!

    It feels weird to be going to a different thread, as I always go to the one related to the K1 visa. I be leaving for the US in a week, and it'll be in California.

    I have read that once you get there, you should wait for a week, then get a SSN. Get married, and have a name change.

    Is it possible if I just get my SSN after we get married? Is this allowed? If so, what are the advantages and disadvantages of applying for a SSN immediately, rather than waiting after marriage?

    This question has probably been asked before, but I don't know how to search for it. :P Thank you!
