K1 to AOS: vaccination supplement not in sealed envelope

Timeline

My husband came to the US from Turkey in September on a K1 visa. Somewhere between here and there, he LOST his DS-3025!! I sent in the AOS paperwork without it (he got all his vaccinations as part of his medical exam and I was hoping all the required info would be in his envelope that was collected at POE). Now I am sure we are going to receive an RFE...but what can I do? How can I get another DS-3025 now? Maybe the hospital in Turkey where he had the physical exam could send us another one? Does anyone have experience with this?? I hope he won't need to have all those vaccinations all over again!!

Just as a bit of background to try to shed some light on this issue..... Some countries allow K non-immigrant visa holders to defer the vaccinations until after they arrive in the US, which is why they request the I-693A with the AOS submission. If the beneficiary's vaccinations were completed as part of the visa process, they are not required to submit the supplemental vaccination form because all that information is enclosed in the brown envelope. This is not to say that someone processing the I-485 will not request the information in an RFE. As we all know, not everyone employed by USCIS has the same understanding of the rules and regulations applicable to K-1 visa holders.

In my own case, I submitted a copy of the DS-3025 and was approved without a problem.

01/26/2010 - Check cashed

01/28/2010 - Received NOA, GC extended for 1 year

02/25/2010 - Biometrics taken

04/23/2010 - Conditions lifted! :)

05/01/2010 - Ten-year GC received...on hubby's birthday! Yay!
