For the vaccination form, you can download it in the USCIS website if you want to bring your own but usually the USCIS civil surgeons has a copy of it already.

You can do it by mail within 10 days if you have your DS3025 vaccination form copy from the US medical exam. And for $10! Just call her number, you may have to leave a message, but they call right back within the day or minutes, then tell them what you need done and they will instruct you with what they need from you next.
