Supplemental Sheet

So can someone give me an example of what a supplemental sheet should have on it Should it just be titled supplemental sheet to I-129F and then on the original forms say see supplemental sheet for full answer or something ... and then on the supplemental sheet just have the Part of the form and the question number with the question copied and then the answer following or what ?? Also do I need to put down the name and such or what help please

I would just refer to the supplemental sheet at the question and then add a sheet with the question at the top of the page and with my supplemental information. I had several supplementals for the I-129 and the G325
