But by th time we were applying for our case I was pregnant so we didn't put her name in there but my fiance said that I was pregnant that time

now i'm waiting for Packet 3 and I have to bring her to get the medical exam soon right

so what i ask is if she needs a photograph too?

4 months old baby? how and anyone has a description about this from USCIS I cannot find nowhere abut the baby's photograph only for adult.

I take it the person you refer to as your fiance is a US Citizen. If so, it is very likely your baby needs a US passport instead of a visa because children of US citizens almost always qualify to be US Citizens regardless of the marital status of their parents at the time of birth.

thank you for your help, my fiance flew to Thailand when the baby was born but he didn't bring her to US Embassy to get the US passport so she is carrying Thai passport right now. And my fiance might not be able to come back here to make her one. so she is Thai for now til we get married there.

Getting married in the US will not make the daughter a US Citizen. If the USC must be present in Thailand to arrange the US Passport then I suppose the Citizenship can be obtained easily enough after arrival. The Consulate needs to be made aware there is a child wishing a derivitive K2 visa, so they will send a Packet 3.
