Did anyone have to pay the civil surgeon for their I-693. The Dr.'s Office is telling me that INS requires to have another medical with the civil surgeon.

Anyone have this expierence? :help::help:

If the medical is not older than one year you do NOT need another medical. What you need is the DS3025 vaccination worksheet transfered to the

I-693 and the fee for that varies, so find a cooperative CS that charges a minimal fee, around $ 35 or so is reasonable.

We paid to have the I-693A completed by the civil surgeon which included a tetanus shot. It cost us $70 and we received the form in a sealed envelope plus they gave us a copy for our records.
