Hi, for the i-751 form I have 3 questions:

 

1) Should I put NA on all of the blank spaces? I am using Adobe Reader and it is hit or miss whether it lets me put it in.

 

2) Page 6 and 8 have a weird prefilled signature (pictures attached). It shows my first 2 initials, then my last name. However, my wife has no middle name and it just puts "N" for the middle initial. Not sure if I should leave that as is.

 

3) It is only asking for addresses my wife has stayed at since having permanent residency. Should I include the address we were at for only a few months when she first got here in the additional information section? Technically she did not have permanent residency status t that address yet.
