Not only a good idea it required....

From the I-129F form:

Using a separate sheet(s) of paper, attach informationrelating to the conviction(s), such as crime involved, date of conviction and sentence.

It's required if it's a conviction. Who knows what the status of this 'list' is.

That's right. The list is not a conviction...at least not in NY. As I briefly outlined in my original question, a finding of having abused a child (called an "indication" in NY) does not carry with it any of the issues associated with a conviction -- either criminal (jail time, fines, a record, etc) or civil (removal of children, being monitored by social services, etc) -- unless it is taken to criminal or family court (which is dependent upon many variables which go beyond the scope of my original question). About the only thing it does is to potentially prevent you from working in the childcare field or adopting a child. Hence, I am pretty confident that leaving the box unchecked would be the correct answer.

No, it is required if you answer "yes" to the question...
