Confusing question i-485 visa in old passport

Hi everyone, I am US citizen, and I am filing i-130 together with i-485 for my mother, she is in USA now and have a question in i-485 part 1. She has two passports, one is new and one is invalid but it contains visa. There is a question in form i-485 "Passport Number used at last arrival", so I indicated her new passport and then they ask a "visa number from this passport", should I write here her visa number from old passport or leave it empty, I am confused?

It's fine either way.  I would write the existing visa number from the old passport.

I'd think she used new passport to enter the US but along with visa from old passport. You'll likely need scans for both passports in the packet.

yes, she used a new passport and on her form 94 is also a new passport info but with old passport's visa. Should I then indicate in a cover letter why did I attached a copy of old passport?
