Next steps after NOA2

We got NOA 2 for our K1 visa on 4/21. We are still waiting on the NVC number. In the mean time, would it be a good idea to start a draft for the DS-160 at around 45 days in anticipation? I saw that it can be saved for 30 days.

My fiancee is in England. When the DS-160 is filed does she need to schedule the medical and interview herself or do they do that and she'll attend at the give times? Is it possible to make an appointment before filing the form?

We have received NOA2 and are waiting for the NVC to receive our case. When the embassy receives the case will they schedule the interview or is that something we can do? Our embassy is in London, England.
